Dr. Loth’s office is environmentally friendly and we strive to be a “paperless” office. Please visit our Patient Login here or using the link below to complete both the Patient Registration and Medical History forms before your office appointment. When your treatment here is complete, a treatment letter along with your digital x-rays will be sent to your referring dentist.
The office is open Monday through Thursday from 7:00 am until 4:00 pm. We will attempt to schedule your appointment as soon as possible. If you are experiencing significant pain and do not feel like you can wait for a scheduled appointment, please inform the staff. Every attempt will be made to see you the same day.
Due to the nature of this practice, we see emergency patients regularly. We make every effort to remain on schedule, but occasionally there will be a slight wait. Please be prompt for your appointment as a courtesy to our other patients. We reserve the doctor’s time exclusively for you. If you must reschedule, please give us 24 hrs. notice if possible.

We participate in Delta, Guardian, Aetna, and Cigna dental networks and can work with most major providers. Our office will file your insurance claim as a courtesy to you, and we will attempt to verify all coverage before treatment. Benefits may vary according to plan. Health related information is kept confidential and released only to in cases of treatment (including direct or indirect by other healthcare providers involved in my treatment, or obtaining payment from third party payers (insurance company). No patient records are released without written authorization from you, or as enforced by law.
